Payment Programs

*** All co-pays for insurance are due at the time of service. The remainder of the fee is due upon discharge. Insured patients may qualify for the Sliding Fee Scale.

*** Quality of Life accepts Medicaid, Medicare and most private insurances. Sliding Fee Scale Discounts based on income and family size are available for uninsured patients upon eligibility approval (See Patient Information).


Quality of Life Health Services, Inc. is a
non-profit organization committed to the delivery of quality patient care. Services are delivered to each individual regardless of ability to pay. It is the policy of Quality of Life Health Services, Inc. to offer its patients a sliding fee schedule based on total household income and family size. Individuals will not be denied or refused services because of inability to pay.

A nominal fee of $15 is required for each medication received through the Prescription Assistance Program. Eligible patients receive up to a three-month supply of most medications approved under the program. Prescriptions, income verification and tax information (if applicable) are required in determining eligibility.

The Quality of Life Pharmacy is located inside the Quality of Life Health Complex and at the Roberta O. Watts Medical Center for your convenience. There are also pharmacies located at Anniston Quality Health Care, Cleburne Quality Health Care and Tuskegee Quality Health Care. Medicaid is accepted at the Quality of Life pharmacies. Quality of Life pharmacies are enrolled in the following Medicare Part D Plans: Aetna, AARP, Caremark, First Health, WellCare, United Health Care and Silver Script.
Medicaid is accepted for exams and glasses. Medicare is accepted for exams only. Other commercial insurances (with vision component) are accepted. Changes for vision services are: $60 eye exam, $80+ glasses.

Most dental insurance is accepted. Medicaid is accepted. Dental clinics are located at the Quality of Life Health Complex, Anniston Quality Health Care, Blount County Quality Health Care, Talladega Quality Health Care, Clay Quality Health Care, Cullman Quality Health Care, Dekalb Quality Health Care, Tuskegee Quality Health Care, Cleburne Quality Health Care, Cherokee Quality Health Care, Gadsden Family & Student Health Solutions, Quality Health Express and the Susie E. Allen Health Center.

For those below 100 % of the Federal Poverty rate, a $15 nominal fee has been established for public housing residents and persons living within one (1) mile of any Greater Gadsden Housing Authority Development. This nominal fee promotes access to primary care.

TOLBERT CLAIMANTS - The Tolbert Plan will end September 30, 2016.
Tolbert Claimants receive medical visits, routine labs, X-rays, and basic dental through the Tolbert Health Plan administered by CTIA, Inc. Insurance will be billed prior to billing CTIA, Inc. Medical services are capped at $ 1,750 annually and dental at $ 850 annually. Tolbert contractually established co-pays apply for dental and pharmacy services.
