Licensed Practical Nurse (LPN)
Assistant to physicians and other professionals in the provision of primary care. Responsive commitment to the needs of patients and ability to deal politely with public even under stress.
Qualifications: Graduated from an accredited program in practical nursing; current registration to work as a LPN in the State of Alabama; At least one year of clinical experience in an outpatient setting is preferred.

Maternity Care Coordinator
The Maternity Care Coordinator ensures that intake procedures are performed on all new Maternity Care Program participants in assigned districts and that medical, psychosocial and social needs are assessed, service plans are developed, information and education are provided, and that recipients are tracked throughout their pregnancy and postpartum period. The Maternity Care Coordinator ensures that all subcontractors in assigned districts are aware of Care Coordination services and patient care is coordinated with each doctor.
Qualifications: Bachelors of Social Work, Master’s of Social Work, LPN certification, or RN certification from an accredited school required. Two years of experience in a health care setting is preferred. Appropriate licensure in specific discipline is required. Must have basic computer skills.
Substance Use Patient Navigator
The SUD Patient Navigator is responsible for promoting patient navigation of Substance Use Disorder (SUD) resources both in-house at QOLHS and via community resources. The individual will perform SUD screenings to determine if those patients who are appropriate for routing into internal and external SUD programs. The SUD Patient Navigator will assist with prevention counseling, document education and link patients to the appropriate level of services. The SUD Patient Navigator will promote patient adherence to a treatment plan established to promote rehabilitation and recovery from addiction. The navigator will also promote primary care and specialty access for patients, as needed. The SUD Patient Navigator will follow up on the team managed approach to SUD treatment, maintaining on-going communication with patients within the QOLHS SUD program and assisting in coordination of care, which will lead to success in improving patient outcomes.
Qualifications: Requires an LPN nursing degree or bachelor’s degree in Social Work. Experience in a clinic-based setting. Must be willing to travel between locations, if needed. Experience working with patients diagnosed Substance Abuse disorder. Ability to present information concisely and effectively, both verbally and in writing.
Risk and Quality Management Manager
The Risk and Quality Management (QM) Manager is responsible for managing and analyzing risk management and performance improvement data, conducting risk management and performance improvement education programs; assists with the review and formulation of policy or organizational changes and making recommendations for final approval; performs these functions with the objective of controlling and minimizing loss to protect the assets of and improving performance activities of the corporation.
Qualifications: Bachelor’s degree as a Licensed Social Worker, Registered Nurse, Licensed Practical Nurse. Minimum of two (2) years experience required in industry and healthcare setting. Quality Assurance Management experience preferred. Strong verbal, written, analytical and persuasive skills, Must have basic computer skills; computer skills as needed for data management and analysis.
