Payment Information

Quality of Life accepts Blue Cross Blue Shield, Medicaid, Medicare, and most private insurances. All co-pays and deductibles for insurance are due at the time of service. No one will be denied access to services due to inability to pay; and there is a discounted/sliding fee schedule available based on family size and income.

Sliding-Fee Discount Program

Uninsured and Insured patients may be eligible to pay reduced fees under the Sliding-Fee Discount Program. Eligibility is based on income and family size. A Patient Service Representative at your health center can provide more details.

Pharmacy Program

We participate in the Federal 340B program. Our pharmacies are enrolled in Medicare Part D Plans, Medicaid, and most major insurances.

Vision Services

Medicaid is accepted for exams and glasses. Medicare is accepted for exams only. Other commercial insurances (with vision component) are accepted. Discounted fees are available for uninsured patients.

Dental Services

Most dental insurance and Medicaid are accepted.
