Quality of Life accepts Blue Cross Blue Shield, Medicaid, Medicare and most private insurances. All co-pays for insurance are due at the time of service. The remainder of the fee is due upon discharge.
Sliding-Fee Discount Program
Uninsured and Insured patients may be eligible to pay reduced fees under the Sliding-Fee Discount Program. Eligibility is based on income and family size. A Patient Service Representative at your health center can provide more details.
Pharmacy Programs
We participate in the Federal 340B program. Our pharmacies are enrolled in Medicare Part D Plans, Medicaid, and most major insurances.
Prescription Assistance Program (PAP): A nominal charge of $15 is required for each medication received through the Prescription Assistance Program. Eligible patients receive up to a three-month supply of most medications approved under the program. Prescriptions, income verification and tax information (if applicable) are required in determining eligibility.
Vision Services
Medicaid is accepted for exams and glasses. Medicare is accepted for exams only. Other commercial insurances (with vision component) are accepted. The nominal charge for vision services is: $60 eye exam + plus cost of glasses (if applicable).
Dental Services
Most dental insurance and Medicaid are accepted.
