Quality of Life Health Services, Inc. houses three (3) Medicaid eligibility workers:

  • One (1) at the Quality of Life Health Complex
  • One (1) at Anniston Quality Health Care
  • One (1) at Talladega Quality Health Care

The New Patient Scheduler allows you to set up your first appointment at any Quality of Life Health Services, Inc. location for medical and optometry services.
